The feedback activity module enables instructors to create a custom survey for collecting feedback from students using a variety of question types including multiple choice, yes/no or text input. Feedback responses may be anonymous if desired, and results may be shown to all students or restricted to instructors only.

Feedback activities may be used:

  • For course evaluations, helping improve the content for later participants
  • To enable participants to sign up for course modules, events etc.
  • For guest surveys of course choices, school policies etc.

Instructors can add a Feedback that can be used to survey their students. This can also be used to test basic knowledge without holding the student accountable with a grade.

Surveys for Mid-Quarter Evaluations

A mid-quarter course evaluation can provide valuable information, which allows the instructor to modify their course or teaching methods to improve learning and student satisfaction.

  1. Select Turn editing on.
  2. Click on the Add an activity or resource link.

  3. Choose the Feedback tool.
  4. Click the Expand all link to view all the setting boxes
  5. expand

  6. Under the General section, type the Name (required) of the assignment into the text box.
  7. Type the Summary (required) into the text box.

    • Enable the use of an open and close date. If enabled, choose the appropriate dates from the pull-down menus.
    • Record user names allows the instructor to have an anonymous survey or not.
    • Allow multiple submissions allows students to submit Feedback an unlimited number of times.
    • Enable notification of submissions sends em ail to the instructor each time a Feedback has been submitted.
    • Auto number questions if allowed will number the questions.
    • Show analysis page allows students to see the Feedback results.
    • Completion message is displayed with the results.
    • Link to next activity: After submitting the feedback, a continue button is displayed, which links to the course page. Alternatively, it may link to the next activity if the URL of the activity is entered here.
    • Group mode allows the groups that have been created in the course to complete the quiz together.
    • Grouping, which is shown under the advanced options, further combines the groups into larger sets. This allows the collections of groups to all complete the quiz collaboratively.
    • Visible sets whether or not the groups will be able to see one another in the activity.
    • An ID number can be set which allows for easy reference of the assignment in the gradebook.
    • Restrict access allows the instructor to set a condition on the assignment until other assignment(s) are completed: Add restrict access image

      • Date: Access can be restricted from or until a certain date and time.
        Note: ALL dates must be changed when copying courses. [We recommend that you do not use the DATE option in Restrict access. Dates can be set above within the Availability.]
      • Grade: determines grade conditions which must be met in order to access the activity.
        Note: Multiple grade conditions may be set if desired. If so, the activity will only allow access when ALL grade conditions are met.
      • Group: If groups are used in the course, it is possible to restrict the activity to a certain group.
      • User Profile determines access based on fields within the student's profile, such as department or email.
      • Restriction Set allows you to set nested restrictions (of those listed above).
  8. Choose either Save and return to course or Save and display.

Adding a question

There are a total of five question types and four tools.

Available questions types:

Captcha: requires the student to copy the distorted text in a text box so that the survey isn't spammed by computer programs.
Information: allows the instructor to display one of three types of information (time, course and or category).
Label: a Moodle label to separate sections.
Longer Text Answer: text box students can write long answers into.
Multiple Choice - Multiple Answer: one correct answer or many.
Dropdownlist (rated): Likert scale (5 loved it / 4 liked it / 3 OK / 2 didn't like it / 1 hated it) Pick the answer from a dropdown list.
Numeric Answer: number for an answer (range)
Multiple Choice - single answer: Radio buttons only one answer
Multiple Choice (rated): Likert scale (5 loved it / 4 liked it / 3 OK / 2 didn't like it / 1 hated it). Pick the answer from a list of choices.
Short Text Answer: type in a single line answer.

  1. Click on the name of the Feedback activity that was previously added.
  2. Click the tab at the top of the page labeled Edit questions.
  3. ?

  4. Choose the type of question from the pull-down menu.
  5. Click Save question button.

Previewing a Feedback tool

Instructors can preview the Feedback tool that they have created in order to test the functionality of the questions. This can also be used to check the layout of the Questionnaire.

  1. Click on the name of the Feedback tool activity that is going to be previewed.
  2. Click on the tab at the top of the page that is labeled Preview.
  3. Answer the questions as the students are expected to.

  1. Click on the Edit button, then the Edit settings link.
  2. Click either Save and return to course or Save and display.

Feedback tool permissions

The Feedback tool permissions are established by the system's administrator. The permissions are given to the various roles in order to grant them the ability to perform actions within the activity. The instructor can view these permissions with these steps:

  1. Click on the name of the Feedback activity.
  2. In the Administration block, click the Permissions to view all possible capabilities pertaining to the Questionnaire, and which roles have access to each capability. Alternatively, click the Check permissions button specifically view an a list of all enrolled individuals in the course.
  3. Click on the user's name to check and then click Show this user's permissions.
    Note: Type the user's name into the search box in order to refine the display and more easily locate the intended user.
  4. This page lists all of the various actions that are capable within the activity. The list will tell whether or not the user is capable of each action.

Viewing Feedback reports

  1. Click on the name of the Feedback activity.
  2. Click Logs in the Administration block.
  3. Note: This page displays the activity that has occurred recently on the Questionnaire.
  4. It is possible to filter the report further by making the appropriate choices from the pull-down menus and then clicking Get these logs.

Enabling email notifications

It is possible to receive email notifications that will tell the instructor whenever the Questionnaire is completed.

  1. Click on the name of the Feedback activity.
  2. Click the tab at the top of the page that says Advanced settings or click Advanced settings in the Administration block.
  3. Scroll to the bottom of the page to a box that is labeled Email.
  4. Enter the email address that will receive the notifications.
  5. Click either Save and return to course or Save and display.
